Shift: Monday - Friday, 9:00 A.M. - 5:00 P.M.

Key Responsibilities:

  • Holistic Assessment: Review initial assessments and identify issues related to employment, education, healthcare, social connectedness, and criminal justice, ensuring a comprehensive understanding of clients' needs.

  • Integrated Care Coordination: Coordinate outpatient services for mental health and substance use disorders, facilitating seamless access to vital resources and support networks.

  • Collaborative Communication: Communicate regularly with referral sources and primary care providers regarding client progress and transition planning, fostering collaboration and continuity of care.

  • Logistical Support: Assist in scheduling treatment and arranging transportation for clients, removing barriers to access and promoting engagement in care.

  • Personalized Treatment: Develop and evolve individualized treatment plans, tailoring interventions to meet the unique needs and aspirations of each client.

  • Advocacy and Linkage: Advocate for clients and link families with community services, empowering individuals to navigate and access essential resources for their well-being.

  • Crisis Intervention: Provide crisis intervention as necessary, offering immediate support and stabilization during challenging circumstances.

  • Documentation Excellence: Ensure all services provided are documented in accordance with standards, maintaining accurate and thorough records to support continuity of care and regulatory compliance.

Education and/or Experience Qualifications:

  • Bachelor's Degree in a human service field (e.g., social work, psychology, counseling) or related field.

  • Knowledge of case management and rehabilitation principles.

  • Strong communication skills, both verbal and written.

Required License/Certification:

None.

Additional Qualifications:

  • Ability to evaluate services and provide input.

  • Familiarity with legal requirements and court procedures related to mental health and substance use disorders.
